Auditorium Facility Use Form

This form is to be filled out only by Darien Public School employees after their event has been approved and added to the auditorium calendar.
    Please indicate where students should report upon arrival. If different grades or instruments should report to different rooms, please indicate such information here.

    Please descibe your audio/microphone needs, microphone locations and any special requests.
    If you plan to use projection during your event, please describe your needs (powerpoint, pictures, video, etc.) and provide links (if applicable) to any videos you plan to show.
    Please describe any special audio needs required for your performance. If you desire certain colors on the cyc during certain songs, please list that here.
    If you have any additional requests for your event, please list them here.
